New Patient Information
We are committed to providing the very best neurosurgical care possible. We would like you to know a few things before your first visit to help make your experience the best possible.
Do you need a referral to make an appointment at CBSI?
Depending on your insurance plan, you may need a referral from your Primary Care Physician (PCP) to be seen by a specialist. If your plan allows you to see a specialist without first going to your PCP, you may make an appointment with us.
Please note, your first appointment will be with a Physician Assistant in our practice unless you already have imaging that was done within the last 12 months.
What do you need to know for your office or virtual visit?
It is a good idea to arrive 20 minutes before your in-office visit to fill out all necessary forms. If you would like to fill out forms ahead of time, please download and complete the New Patient Packet here. Once completed, you may email it to CBSI@cbsi.md.
We do prefer to see you in person at one of our locations as a new patient but if you are unable to be seen in person due to distance, ride availability, etc., we can do most evaluations virtually.

What do you need to bring or have available for your first visit?
You should bring:
a government issued photo ID,
insurance card,
imaging if you have it including MRI, CT, X-rays*
a list of other prior treatments,
a list of questions for your provider,
a friend or family member for support!
*If you have any recent imaging (within the past year) including MRI, CT or X-rays, you should bring a copy on a CD. We are able to access imaging from Invision Sally Jobe, Health Images, Touchstone and any imaging done at Centura or HCA Facilities.
Can you see a Physician for your first visit?
As a new patient, you will likely need to see one of our Physician Assistants prior to seeing a surgeon to discuss your condition and treatment options, unless you were referred by your PCP and have relevant imaging and conservative therapy. Our PAs are able to provide imaging orders and PT orders as well.
