Medical and billing records can be requested by leaving a voicemail at (303) 783-8844, Option 5 or sending an email to cbsi@cbsi.md. Please note, for patients requesting medical records, you must email a completed release of information form along with a copy of your driver’s license for verification.

Patients can reach the billing department by phone Monday through Friday, 8am to 5pm.
